How to Become a New Client

All appointments are booked by phone so we can make sure you're getting the right type of visit and timing for your pet's needs.

Gather your pet's previous medical records.
Please bring them to your appointment, or have your prior veterinarian email them to us at records@kentmerevet.com. Current medications and supplements are helpful to bring as well.

Fill out the New Client Information Form below.
This gives us the basics we need before your visit so your appointment can start smoothly.
